Medication Reminders & Monitoring

We help set up a simple, reliable routine so doses aren’t missed or duplicated. We can align scripts, create a pill-time schedule, and use reminder tools (pill boxes, phone prompts). Care Workers provide reminders and observation; Nurses may administer medications only within scope and on clinician orders.

Vital-Signs Checks (BP, Glucose, Cholesterol, Temp, Pulse, SpO₂)

Regular observations help spot problems early. We record readings and watch for red flags, following your clinician’s thresholds and action plans.

  • Blood pressure with positioning & resting protocol
  • Glucose finger-prick & symptom checks (per plan)
  • Cholesterol rapid test (if available/arranged) or clinic referral
  • Temperature, pulse, respiratory rate, SpO₂ monitoring
  • Trend logs and family/clinician updates as agreed
Red-flag examples
  • Very high/low BP or glucose outside plan ranges
  • SpO₂ drop, chest pain, severe breathlessness
  • Fever with confusion or uncontrolled pain

Note: Where home devices aren’t available, we can help source or arrange clinic checks.


Basic Wound Care & Dressings

We support basic wound-care routines per your clinician’s orders. Care Workers assist with hygiene and observation; Nurses perform dressings and clinical tasks within scope.

  • Clean technique, simple dressings, and scheduled changes (nurse-led)
  • Pressure-injury prevention: turns, skin checks, cushions/mattress guidance
  • Signs of infection & escalation (redness, heat, exudate, fever)
  • Supply checklists and pharmacy coordination (by arrangement)
Bring to assessment
  • Doctor/clinic wound-care orders
  • Current products & frequency
  • Photo (if appropriate) to track progress

Coordination with Doctors & Clinics

With your consent, we liaise with your clinicians to keep home care aligned to your plan. We can help book appointments, prepare notes, and provide updates to reduce readmission risk.
