- Diagnoses, current medications, therapy notes (PT/OT/SLT) if available
- Mobility status & equipment (wheelchair type, hoist/slings, braces, splints)
- Transfer method & weight-bearing status; preferred positioning
- Skin/pressure-risk history; current wounds & dressing orders (if any)
- Bowel/bladder routines (catheter, stoma, continence plan)
- Feeding plan & any swallowing risks; allergies
- Seizure or behaviour support plan; triggers & calming strategies
- Daily schedule goals (school/work/therapy/community); transport needs
- Preferred languages & communication supports (AAC/PECS/devices)
Home setup & equipment
We can advise on ramps, grab bars, shower chairs, transfer boards, pressure-relief cushions/mattresses, and safe-path layouts.
Clinical scope: Care workers handle non-clinical tasks and reminders. Nurses perform clinical care per training and doctor’s orders.
